If you have started researching hair transplantation in Istanbul, you will see two acronyms repeated everywhere: FUE and DHI. They are often presented as competitors. They are not — they are different tools for different jobs, and the right choice depends on your hair loss pattern, the area you want to treat, and your lifestyle preferences during recovery.

This guide explains, in plain language, what each technique actually does, when each one wins, and why a serious clinic will sometimes recommend a hybrid of both within a single procedure.

The shared foundation: Follicular Unit Extraction

Both Sapphire FUE and DHI begin with the same step: extraction. Individual hair follicles are removed from the donor area at the back and sides of the scalp using a precision micro-punch, typically 0.7–0.9 mm in diameter. The follicles are kept in a chilled holding solution that maximises their viability before implantation.

This is the same in both techniques. What differs is how the follicles are placed into the recipient area.

Sapphire FUE — the open-channel method

In Sapphire FUE, the surgeon opens micro-channels in the recipient scalp using sapphire-tipped blades. The angle, depth, and direction of each channel is chosen to mimic the natural growth direction of the surrounding hair. Once all channels are open, the follicles are individually placed into the prepared sites using forceps.

Strengths

  • Excellent control over angle, direction, and density.
  • Very high graft density possible (up to 55–60 grafts/cm²) on the front line and crown.
  • Sapphire blades create cleaner, smaller channels than traditional steel blades — faster healing, less scabbing.
  • Suitable for all areas: hairline, mid-scalp, vertex (crown), beard.

Trade-offs

  • The recipient area must be shaved (1–2 mm) to expose the channels.
  • Two clear stages — channel opening, then placement — slightly longer total procedure.

DHI — Direct Hair Implantation

DHI uses a Choi pen (sometimes called a Choi implanter): a fine hollow needle pre-loaded with a single follicle. The surgeon presses the pen against the recipient scalp; in one motion the channel is opened and the follicle is delivered into it.

Strengths

  • The recipient area does not need to be shaved in many cases — only the donor area at the back. Patients with longer hair can return to work within days without an obvious "I had a transplant" look.
  • The follicle is exposed to air for less time, theoretically improving graft survival.
  • Excellent for filling gaps in existing hair.
  • Highly suitable for eyebrows, beard, and women who do not want a fully shaved head.

Trade-offs

  • Slightly slower per-graft rate, which limits maximum session size.
  • Density on a fully bald scalp may be marginally lower than sapphire FUE in expert hands.
  • Choi pens require very high operator skill — the technique is quality-of-team-dependent.

Which one is right for you?

A simple decision tree:

  1. Are you a Norwood 5–6 with a fully bald top? Sapphire FUE in two sessions, or a single mega-session of 4,500–5,000 grafts. DHI alone may not achieve the density you want in one trip.
  2. Are you mostly thinning, with hair you do not want to shave? DHI. The "no-shave" option means you can return to public life quickly.
  3. Are you a woman with frontal thinning? DHI almost always — preserves your existing hair length.
  4. Eyebrows or beard? DHI. The Choi pen places follicles at the very fine angles required.
  5. Are you younger than 25? Wait. Hair loss patterns are still maturing. A transplant before pattern stabilisation often produces a cosmetic mismatch in 5–10 years. Use medical therapy (finasteride, minoxidil) as a holding strategy.

The hybrid approach

Patience is the most underestimated part of a transplant. The "before and after" photos that excite you on Instagram are 12–18 months apart.

What to avoid in a clinic search

  • Quoted prices that include flights. This is a marketing tactic that compresses your treatment to the cheapest team.
  • Unspecified team. Ask: who is opening the channels? Who is placing the grafts? What is the surgeon-to-technician ratio?
  • No personalised plan. Any reputable clinic should look at your photos before quoting graft numbers.
  • Pressure to add grafts on the day of surgery. Your treatment plan should be agreed and signed before you arrive.

How many grafts will I need?
